We have an opening in the Mobile area for a Licensed Certified Social Worker or Licensed Clinical Psychologist with Geriatric patient experience to provide mental health services within long-term care/nursing facilities. Responsibilities: 1. Perform client evaluations and provide diagnosis. 2. Formulate plan of treatment which promotes development and maintenance of individual goals. 3. Ability to use analysis, experience and judgment to make effective decisions. 4. Provide crisis intervention and/or family therapy. 6. Assess and monitor patient to evaluate success of treatment and adapt intervention as needed. 7. Maintain documentation of patient services via our electronic medical record (EMR) platform, ensuring compliance. 8. Interface and collaborate with facility interdisciplinary team regarding patient treatment planning, progress and observations.
